MDS Nurse

POSITION SUMMARY:  Name of Facility  is looking for a MDS Nurse who is flexible, reliable, well organized and a team player.  The primary purpose of the MDS Nurse is to plan and organize the MDS/RAI process in coordination with the RAI/PPS Coordinator.  This position assures compliance with all State and Federal MDS transmission requirements.  The MDS Nurse is responsible for collaborating with Clinical and Administrative staff regarding issues relative to the Resident Assessment Instrument (RAI) process.

The Wellness Nurse is responsible for coordinating and implementing resident care in accordance with the Care Plan and the standards of the organization, always fostering residents’ independence and freedom of choice. Provides guidance to the Resident Assistants. All responsibilities will be conducted in a manner that respects resident’s dignity and privacy. The Wellness Nurse maintains resident care plans, supports residents and family members through transitions of care and supports the Director of Assisted Living. iN2L Program Partnership Specialist

It’s Never 2 Late (iN2L) is seeking a talented and competent Program Partnership Specialist to work directly with the Director of Research and Program Partnerships to assist senior living providers procure funding to launch the iN2L Program. The objective of the iN2L Program is to support projects with quantitative and qualitative objectives that improve the quality of life and care of senior living residents by enhancing activity, clinical, restorative care and rehab offerings. Internally the Program Partnership Specialist supports the day-to-day proposal development activities for Requests for Applications (RFAs), grant proposals and post-submission procedures for CMS funding opportunities in various states. Job Purpose
• Assist with pre-proposal/proposal planning, organization, scheduling, monitoring, and controlling the proposal throughout the lifecycle.
• Assist with contract compliance monitoring to ensure contract deliverables and requirements are met; proposes revisions to policies and procedures.
• Research federal and state regulations and statutes, encompassing such topics as Civil Monetary Penalties, HIPAA, Medicaid and compliance, and analyzes their applicable impact to the iN2L Program contract requirements.
• Conduct informational webinars for grant applicants interested in launching the iN2L Program.
• Attend selected state conferences to provide information of the iN2L Programs and CMS funding opportunities.
• Review and edits drafts for readability, clarity, grammar, spelling, punctuation, sentence structure and formatting.
• Identify, assess, resolve, and/ or escalate issues as needed.
• Review proposals to ensure compliance with RFP requirements; identify potential concerns or challenges.
• Support proposal production including data collection, input, editing, and proofreading.
• Input, update and maintain customer relationship management (CRM) system.

Qualifications

• B.S. or B.A degree
• Proven consulting, and/or customer service/support skills
• Experience with grant application procedures and grant-funded programs processes preferred.
• Experience with government contracting preferred.
• Knowledge of CMS guidelines and regulations a plus.
• Strong oral, written, and presentation skills
• Ability to organize people and time effectively
• Experience in senior living industry highly desired
• Knowledge of CRM software (ZOHO) preferred
• Ability to travel up to 25%
